dm+d changes going live in January/ February 2023

Dear colleague,
An additional dm+d historic code file will be available in TRUD from 16 January 2023.
Current dm+d VTM content will link to “contains only X” and Medicinal Products in SNOMED International Edition from 13 February 2023.

ICD-10 Books and Electronic Versions of the Classifications

Dear Colleague
The World Health Organization (WHO) are no longer printing the ICD-10 5th edition volumes in book format. The Stationary Office (TSO) have reported these volumes are not available to purchase. As a result we will discuss with WHO to investigate any alternatives.
The ICD-10 5th Edition and OPCS-4.9 classifications are freely accessible via the web-based NHS Digital Classifications Browser where you can browse and search content. Links are provided from the Tabular List chapter, category or subcategory level to the associated National Clinical Coding Standard; the full set of National Clinical Coding Standards can also be viewed or downloaded via the browser.
Modern classifications will take advantage of latest technology and have digital capability to be used in digital health applications and information systems. The World Health Organization have taken this leap with ICD-11 and we will follow suite in a future revision of OPCS-4 moving away from providing the OPCS-4 classification as a hard copy book.
We know that for many of you this will not be an issue, but we do recognise the difficulties this may present some coding departments. So, we are looking at how we can best support you in this transition, and over the coming months we will:
- Conduct a survey to understand the level of demand for books, why books are still needed, and the local technical constraints in adopting digitally enabled classifications in day-to day-coding
- Undertake user centred research with:
- Coding trainers to understand the logistical issues in delivering training without hard copies and how the training and materials can be changed to accommodate the classifications browser
- The Classifications Browser User Group to understand how they use the Browser, tips, best practice and future functional improvements
- IHRIM in relation to the future use of the Classifications Browser as part of the NCCQ Examination.
- Deliver live user awareness sessions on how to make best use of the Classifications Browser
- Publish a roadmap for the transition to fully digital classifications, and the deprecation of the eVersion.

Data Dictionary for Care (DD4C) Migration to Delen

Dear colleague,
The DD4C library of published subset metadata has been migrated to Delen, with effect from 08 November 2022.
Anyone attempting to access the original DD4C website will automatically redirected to the new DD4C page.
DD4C functionality includes:
- published subset metadata content freely available to users without the need to register for an account
- content presented in two views:
- summary view (shows six Subset Metadata data attributes)
- detailed view (shows all Subset Metadata data attributes)
- content can be sorted in either view, on up to three attributes
- content remains searchable
A significant area of change of functionality is how DD4C content is maintained. Users can no longer maintain content through the website but must submit a request for change to the Terminology and Classifications Delivery Service. Details on how to do this are available on our Requesting Changes to DD4C Content page.
What hasn't been migrated?
- Unpublished Subset Metadata
- Archetype Metadata
- Data Set Metadata
- Associations between one Subset and another
- Related links to other websites (e.g., NHS Data Model and Dictionary)
- User accounts

Publication of the OPCS-4.10 System Data Files
Published: November 2022
Dear colleague,
The OPCS-4.10 System data files and specifications (including XML format), to support OPCS-4.10 implementation on 01 April 2023, are available for download from the Technology Reference data Update Distribution (TRUD) service subject to registration and acceptance of the Licence Agreement.
The data files are designed for NHS information departments and /or NHS system suppliers to upload into PAS systems. They are not designed or intended for training purposes.
Implementation guidance and other information and resources for OPCS-4.10 are available on the OPCS-4 page on Delen.
The Information Standards Notice (ISN) describes the mandate for the NHS and NHS suppliers to implement OPCS-4.10 on 01 April 2023.

dm+d stakeholder engagements to understand impacts of VMP ID code changes

Dear colleague,
The dictionary of medicines and devices (dm+d) is undergoing some enhancements in the near future, one of which are a number (circa 2300) of VMP ID code changes.
In order for us to implement these changes smoothly, we are engaging with users of dm+d data so that we can understand how the code changes will be handled and the volume of code changes that can be processed. NHS Digital colleagues have already started these engagements. If you think that this work will impact you and you have not yet been contacted, please email kerry.frenz2@nhs.net and paul.wright39@nhs.net. A roll out plan of the code changes will be communicated once stakeholders have been engaged.

Register for NHS England Pricing DQ webinar 5th October 12:00 - 1:00
NHS England is running a Pricing DQ webinar that will discuss work being done to monitor the quality of activity data used to inform prices published as part of the national tariff/NHS payment scheme. The project is particularly interested in identifying any change in data quality following the move from activity-based payment to blended payment with a larger fixed component. The session will also discuss the role of prices in the NHS payment scheme. The webinar runs from 12:00 - 1:00 on 5th October and you can register to join here.

Advanced Notice: Interesting excipients authoring in dm+d to come to an end
Further to feedback from dm+d users on the recent ‘Interesting’ Excipient Consultation which ran from 18 April 2022 to 17 June 2022; the dm+d Content/Editorial Committee - chaired by the Department of Health and Social Care - have approved the decision to end the authoring of the interesting excipient information from dm+d.
The timetable and exact details of this change will be communicated in due course through the weekly release notes for dm+d.

OPCS-4.10 update - change to ISN and data files release date
OPCS-4.10 was scheduled for approval by the Data Alliance Partnership Board (DAPB) in September followed by publication of the Information Standards Notice (ISN) and release of the final Summary of Core Changes from OPCS-4.9 to OPCS-4.10 and OPCS-4.10 data files at the start of October 2022. However, these dates have been altered and the ISN and data files will be published at the start of November 2022.
Implementation of OPCS-4.10 remains 1 April 2023.
The Consultation on the Draft Summary of Core Changes form OPCS-4.9 to OPCS-4.10 ended on 29 July 2022. Thank you to everyone who took the time to review the changes and send feedback. We have reviewed the feedback and made a number of changes to the draft changes as a result, and we are currently finalising the content of OPCS-4.10 and producing the data files.

NHS Digital is developing the next update to OPCS-4 (OPCS-4.10) for NHS implementation on 1 April 2023 as part of the three yearly release cycle.
The classification has significant capacity constraints, so a revision is required to address this issue and ensure the classification can continue to accommodate developments in clinical practice. NHS Digital has begun early planning and intends to conduct a more extensive user engagement exercise to ensure that this evolution addresses the medium-term needs of the health and care system in the UK.
The long-term future direction will be reconsidered as interoperability across the NHS increases and will reflect further engagement with international standards providers around the identification of a potential international replacement. It is expected that, in the longer term, there will be a need to move to a new procedure classification that can better take advantage of a context when clinical records are comprehensively and consistently populated with SNOMED CT coded data. Consideration of the strategic replacement will include consultation with the NHS.
The scope for the revision work is restricted to the medium-term revision to OPCS-4. We will publish further details on the expected plan and user engagement as this evolves.
In the meantime, our immediate priority is to deliver the OPCS-4.10 update.
Classifications Browser – ICD-10 Content
The latest development work on the classifications browser means the ICD-10 and OPCS-4 classification and clinical coding standards are now all available in a single online application.
Thank you to the members of the Classifications Browser User Group who dedicated their time, at short notice, to take part in the testing during July.
An updated release of the Classifications Browser is now available.
The main improvement is the inclusion of ICD-10 content to browse, search and link to the relevant ICD-10 clinical coding standards. Additional improvements for this release are:
- Improved indexing with the inclusion of lead terms and modifiers in the Volume I – Alphabetical Index tree
- Incorporation of content changes published in that are displayed as red notes in the eVersion.
Following release of ICD-10 in the Browser, the next phase of work will be to investigate the incorporation of a note taking facility.
SNOMED CT Implementation – User Research Survey
The SNOMED CT Implementation team have launched a short survey relating to a new project they are undertaking on value sets.
A common challenge reported to us is the inability to find and maintain the lists (or value sets) of SNOMED CT concepts required for health and care. Users may create these lists for the following reasons:
- analytics
- data entry
- decision support
- reporting
So, whether you currently create or use value sets or think they may help you implement SNOMED CT in the future, we want to hear from you.
The survey should take less than ten minutes to complete and there is an option to register your interest if you want to be considered as part of our user research panel.

NHS Digital becomes WHO Collaborating Centre
NHS Digital has been redesignated as the UK Collaborating Centre for the WHO Family of International Classifications (WHO-FIC) for a four-year period. This grants NHS Digital an important level of profile, authority and influence, as well as opportunities to collaborate in the WHO-FIC network. The network and its collaboration with other centres and international standards organisations is an important resource and will be vital for the UK as we implement ICD-11.
Delen has grown significantly since its launch in 2017, with a year-on-year increase in the number of page views and returning users.
As a key communication channel, it’s important that we continue to improve the design and curate content, so it remains relevant, up-to-date, and accessible to our users.
Following a significant redesign project, our refreshed Delen site went live at the beginning of September. The site has been given a fresh new look and feel and has been redesigned to better reflect our core products and the services that we provide. Take a look at the new and improved Delen.
Delen ICD-11 pages
As part of our wider Delen refresh, we have updated our ICD-11 pages. The main change you will notice is that the content has been organised around users rather than topics. We hope this will mean you have a better experience and can more quickly find the information that’s relevant to you.
The new ICD-11 pages include an FAQ database containing questions asked by the almost 400 members of the clinical coding community who responded to our recent ICD-11 questionnaire. We have also published an overview presentation setting out our approach to the early preparations for moving to ICD-11 and have created a downloadable interactive catalogue of ICD-11 Stakeholder Profiles and Use Cases. Over the coming weeks, we will publish more tailored content as well as links for you to sign up for interactive online ICD-11 engagement sessions.
We hope you find the content interesting and useful and we encourage you to contribute to the implementation work by reviewing our approach and our forthcoming draft implementation products, which will help ensure our plans are realistic.
SNOMED CT to ICD-10 and OPCS-4 Classification Maps for the 34.1.0 – 10 August 2022 content
There were no updates to the ICD-10 or OPCS-4.9 classification maps in the 34.1.0 – 10 August 2022 release.
The August 2022 SNOMED CT content will be mapped and included in the 35.0.0 - 05 October 2022 SNOMED CT UK Clinical Edition, which will also include the UK maps for SNOMED International Edition February 2022 to July 2022 releases..

Interactive ICD-11 Stakeholder Profile and Use Case Catalogue Published
We have published the first version of our ICD-11 Interactive Catalogue of Stakeholder Profiles and Use Cases. The catalogue is intended to help us identify the stakeholder groups and use cases that need to be considered when developing our ICD-11 implementation approach, impact assessments and implementation plan. As with all our ICD-11 work, we are adopting a "share early" principle, so that our work can benefit from stakeholder input as early as possible. The catalogue contains a feedback mechanism and our implementation approach page provides links to forms where you can suggest additional stakeholders and use cases. Because we are sharing early, there will likely be a higher than normal number of bugs or errors, but we hope that any annoyance caused by this is outweighed by the benefits of having earlier sight and earlier opportunities to provide feedback. We hope you find the catalogue useful.
